Insurance

Dr. Mednick exclusively accepts Cigna and UHC/Oxford Insurance. If you would like to pay out of pocket, her current rates are $545 for an initial evaluation and $195-350 for follow ups depending on visit length and complexity ($195 is the shortest, refill-only appointment).  The maximum interval between visits once you are feeling stable is 3-4 months. Dr. Mednick's office will be happy to help you submit the proper paperwork and coordinate any out of network benefits you may have for reimbursement.  In case you're not sure about your coverage, you can look at the guide below for a sample out of network inquiry or our office can make an inquiry on your behalf.

 

HOW TO INQUIRE ABOUT OUT-OF-NETWORK BENEFITS: 

 

​Call the number on the back of your card and ask specifically for mental health or behavioral benefits. Ask the following questions: 

 

• If Dr. Mednick (NPI# 1548497902) is not in my network, what do I have to pay to see her out-of-network?

• Do I have a deductible? If yes, how much of it has been met to date, and when does the deductible year begin?

• What percentage of my bill will be paid by the insurance company? Is the percentage of my bill paid by the insurance company based on usual, customary, and reasonable (UCR) schedules or on the actual charges?

• Does my policy have an out-of-pocket maximum? If my out-of-pocket expenses reach a certain amount, will the insurance carrier ever reimburse at 100 percent?

• If so, does this mean that my claims will then be paid in full, or will I still be responsible for the difference between my doctor's charges and usual, customary, and reasonable (UCR) rates? Rates are charged for the following CPT codes: 90792, 99213, 99214, 99215, 90836, 90834.

• What is the fax number or address where I can submit my claims for reimbursement?
